I agree to become a Corporate Member of the Pilates Method Alliance, Inc. (PMA), a Florida nonprofit corporation and an organization exempt from federal income taxation under Section 501(c)(3) of the Internal Revenue Code of 1986, with its principal place of business at 8333 NW 53rd Street, Suite 450-4501, Doral, FL 33166.
I understand this is a non-exclusive membership in exchange for a certain contribution.I agree and in consideration of the mutual promises and covenants contained herein, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties, intending to be legally bound, hereby agree as follows:1. TERM. The term of this agreement will commence upon receipt of corporate membership fee and will continue for 12 consecutive months and may be renewed for additional years by mutual agreement of the parties.
2. MEMBERSHIP AND CONTRIBUTION.
A. Membership. During the Term of this Agreement, PMA hereby agrees to identify and acknowledge applicant company as a Corporate Member of the PMA, as permitted in connection with qualified membership payments under Section 513(i) of the Code and the Treasury regulations thereunder. Such identification and acknowledgement shall include the membership benefits afforded the level of membership to which this company applying.
B. Contribution. In consideration for the right to be a PMA corporate member during the Term of this Agreement, applicant company agrees to make a cash contribution equivalent to the level of membership applied for to be paid in full upon submission of this agreement.
C. Benefits. PMA corporate member shall receive all benefits afforded the level of membership to which this company is applying.
3. CANCELLATION POLICY. By checking yes to this agreement, I understand and agree to the following: Cancellations by member must be received in writing by the PMA. Membership fees are non-refundable.
